How to Apply

Please complete the steps below:

1.  Please verify the applicant meets eligibility requirement.

You can find these requirements on the OK DHS website at: http://www.okdhs.org/services/dd/Pages/SW.aspx

2.  Complete the ​DDSD Application​ ​ (if needed).

The form can be filled in online and printed, or you may print it and fill it in by hand. Mail the complete form to:

3.  Fill in New Horizons Worker Application form online or Download the form. Print and fill in the form and mail to:

 New Horizons

 319 W. Oklahoma

 Guthrie, OK 73044

4.  Other items to be submitted for employment consideration:

A.   Current Psychological assessment (within the past five (5) years for private or state funded services; if an individual is going to be receiving waiver          services. DDSD will require a psychological assessment to be done within the last year).

B.  Written authorization, i.e.; Notice of Action Form, Notice of Authorization Form, from DHS-DDSD Admin. Services and/or Oklahoma Health Care             Authority defining the type of service that New Horizons will be reimbursed.

C.  A current medical assessment (within the last year).

D.  OSBI background checks (New Horizon will obtain this).

E.  Guardianship papers, if applicable.

F.  Any other assessments/information applicable.
